New Patients
Please call us at (240)-232-2020 to schedule your first appointment. Our friendly staff looks forward to assisting you prepare for your visit at Maryland Eye Center. Our office participates in medical insurance, so please be prepared to have your insurance card with member ID available prior to calling our office.
Patients can save time during their first appointment by completing their Patient Registration forms prior to their visit. Simply print out the Patient Registration form, fill in the information requested, and bring the completed form with you to your appointment.
Have you previously seen or currently seeing a different provider for your eyes ? Have you had any previous eye surgeries or diagnosed with any eye conditions? Please let us know so our staff can request those medical records prior to your appointment.
